Generating lab requisitions
Create a PDF lab requisition, filled in from the patient record, and fax it to any lab.
A requisition is the paper route. Use it when the lab isn't reachable through Junction or Fullscript, or when the patient is taking the order to a lab of their choice. Ona builds the PDF, and you send it. Status and results don't flow back automatically, so upload the report when it arrives (see Reviewing lab reports).
Creating a requisition
- Open the patient and go to Lab Orders in the Dispensary section.
- Select Order labs → Lab Requisition. If your practice has no lab integration, the button reads New lab request.
- Fill in the New lab requisition sheet:
- Destination lab (required): the lab's name, for example Quest Diagnostics.
- Tests & panels (required): type each test, and press Enter or comma to add it.
- Diagnoses (ICD-10): add codes for medical necessity. The description is optional.
- Collection date, Instructions to the lab (for example, "Fasting required") and Comments.
- Select Generate requisition.
The requisition appears in Lab Orders as Generating, then Send and View appear once the PDF is ready.
Optional details
Two collapsed sections hold details you usually don't need to touch:
- Destination lab details: the lab's address, phone, fax and NPI, kept with the requisition for reference.
- Patient, provider & signature: override the patient's name, date of birth, sex (as printed) or MRN; the ordering provider's name and NPI (it defaults to you); and the signature name (it defaults to the ordering provider).
What's on the PDF
- Header: your practice name, address and phone, with the ordering provider's name and NPI.
- Patient information: name, MRN, date of birth, sex, phone numbers and address.
- Insurance information: primary and secondary coverage (policy holder, group number, member ID), taken from the patient's insurance on file.
- Requisition: diagnosis codes, tests ordered, instructions and comments.
- Electronically signed by the ordering provider, plus their handwritten signature if they saved one under Settings → Profile → Signature.
Sending it
- View opens the PDF, with Download to save it and Open to print it from a new tab.
- Send faxes it. Choose a saved fax contact, or type the lab's fax number (for example +12125551234), then select Send fax. Track delivery under Faxes → Sent.
