Reviewing lab reports

Lab Reports is where results live, whichever way the test was ordered. It sits above Lab Orders in the Dispensary section of the patient record.

Where reports come from

• Junction orders: results arrive on their own, tagged Junction. See Ordering labs through Junction.
• Faxes: lab results that come in by fax are added for you.
• Uploads: anything else, such as a requisition result, a Fullscript results PDF, or a report the patient brings in.

Uploading a report

1. Select Upload report.
2. Drop in the file. PDF, Word, JPEG, PNG and HEIC are accepted, up to 25 MB.
3. Give it a Title (for example, "CMP — Quest, April 2026"). You can also add the Lab / ordering provider, the Specimen collection date and an Internal note.
4. Select Upload & parse.

The report shows Queued, then Parsing, while Ona extracts the values. It then moves to Needs review. If extraction fails (Parse failed), try uploading a clearer copy.

Reading a report

Select a report to open it.

• Original document, on the left, is the report exactly as the lab issued it.
• Parsed values, on the right, are grouped into Needs attention, Normal and Other. Each value shows its LOINC code, the result and unit, and a flag: Normal, Low, High or Critical. Where the lab gave a numeric range, the value is plotted on it.
• Select Accept on a value to confirm it was read correctly.

For Junction reports, the header shows Final or Partial — more pending, and View order takes you back to the order. A partial result isn't final, so don't act on values that are still missing.

Critical results

A report with a critical value opens with Critical result — acknowledge now. An Owner, Admin or Doctor selects Acknowledge to record that it's been seen. For Junction orders, an unacknowledged critical result alerts the ordering clinician again after 15 minutes.

Reviewing and archiving

When you've checked the values, select Mark report reviewed. The bar then shows who reviewed it and when. Junction reports can only be marked reviewed once the final result is in.

Archive moves a report out of the list, for example a duplicate upload. Its values stay in the chart, and Restore brings it back.

Use the filters at the top of the list (All, Needs review, Parsing, Failed, Reviewed) to find what's still waiting for a clinician.

Trends

Trends charts every marker across the patient's reports, so a cholesterol or vitamin D level reads as a line over time, not as scattered numbers. Search for a marker with Search analytes…, and select a point to open the report it came from. Values are matched by LOINC code, which is what lets results from different labs line up.

What patients see

Reports appear in the patient portal under Lab reports. A report shows as Pending review until a clinician marks it reviewed, and then as Results ready.
