Carrying records forward between visits
Bring active chart diagnoses, medications and allergies into a visit from the visit Records page instead of re-entering them.
Use the visit Records page when the visit should include a diagnosis, medication or allergy that already exists on the patient chart. Records that are active on the chart but not yet attached to the visit appear under From the chart.
How do I carry one chart record forward into a visit?
In the left sidebar, click Patients, then click the patient's name. In the patient chart, click Visits, click the visit you are charting, then click Records in the visit sections. Use From the chart to bring an existing chart record into the current visit without creating a new record.
- Find the section you need, such as Diagnoses, Medications or Allergies.
- Under From the chart, find the record you want to include in this visit.
- Select the carry-forward control on that record.
The record is attached to the current visit and remains part of the patient's chart.
How do I carry all records in a section forward?
In the left sidebar, click Patients, then click the patient's name. In the patient chart, click Visits, click the visit you are charting, then click Records in the visit sections. Use Carry all when the visit should include every chart record shown in one section.
- Find the section you need, such as Diagnoses, Medications or Allergies.
- Review the records under From the chart.
- Select Carry all.
Ona attaches those chart records to the current visit. Records already attached to the visit are not shown in From the chart.
How do I add a new record while charting a visit?
In the left sidebar, click Patients, then click the patient's name. In the patient chart, click Visits, click the visit you are charting, then click Records in the visit sections. Add a new record when the diagnosis, medication or allergy is not already listed under From the chart.
- Select Add diagnosis, Add medication or Add allergy in the matching section.
- Search for the condition, medication or allergen.
- Select the item you want, or continue with a custom entry when the dialog offers one.
- Select Next.
- Add the details needed for the record.
- Select Add diagnosis, Add medication or Add allergy.
The new record is saved to the patient chart and attached to the current visit.

What does From the chart mean?
Open a visit by clicking Visits in the left sidebar, then clicking the visit. On the visit's Records page, From the chart means the record is active on the patient's chart but is not attached to the current visit.
Use From the chart as a review list. Carry forward records that matter for the encounter. Leave records there when they should stay on the patient's chart but do not need to be part of this visit.
Allergies shown under From the chart are expanded by default. If the patient has a No known allergies record, it appears as a chart-level statement rather than a per-visit allergy.
What can't I do here?
Open a visit by clicking Visits in the left sidebar, then clicking the visit. On the visit Records page, carry-forward actions are limited in these ways:
- You cannot add, edit, archive or carry records forward when the visit is read-only.
- Carry all appears only when there is more than one chart record in that section.
- No known allergies cannot be carried forward as a per-visit allergy.
- A diagnosis must use a billable ICD-10 code unless you add it as a custom entry.
Common questions
Does carrying a record forward duplicate it?
No. Carrying a record forward attaches the existing chart record to the current visit.
Why don't I see Carry all?
Open a visit by clicking Visits in the left sidebar, then clicking the visit. Carry all appears in From the chart only when that section has more than one record available to carry forward and the visit is not read-only.
Can I edit a carried-forward record?
Yes, when record editing is available for the visit. Open the record pill and use its edit action. The available fields depend on the record type.
What happens if I leave a record in From the chart?
The record stays on the patient's chart, but it is not attached to the current visit.
Can I carry AI-suggested records forward?
AI-suggested records may show review actions on the record pill. Confirm or dismiss suggested records from the pill actions when those actions are shown. See Reviewing AI-suggested records.
