Co-signing a note

Add your signature to an already signed note and review the signature list from the note panel.

Co-signing adds your signature to a note that is already signed. You do it from the note panel in a patient's Notebook or a visit's Notebook.

How do I co-sign a note?

In the left sidebar, click Patients, then click the patient's name. In the patient chart menu, click Notebook. Co-sign from the note panel after you open the signed note.

A signed note is open with the note content above a signature footer.
  1. Select the signed note you need to co-sign.
  2. In the Signed by footer, select Add my signature.
  3. Wait for the button to finish Signing….

Your name is added to Signed by. The note stays signed.

You can also co-sign from a visit notebook. In the left sidebar, click Visits, then click the visit. Click Notebook, open the signed note, then select Add my signature in the Signed by footer.

How do I check who has signed a note?

In the left sidebar, click Patients, then click the patient's name. In the patient chart menu, click Notebook. The Signed by footer appears at the bottom of a signed note and lists each signer.

A signed note is shown in a locked state with signature information at the bottom.

The footer shows the total number of signatures next to Signed by. Each signer appears on a separate row with the signer name, when available, and the signed date and time.

Some signature rows use fallback labels. Former member means the signature is tied to a member who is no longer available. Signed on import means the note was imported as already signed.

What happens when I co-sign?

Co-signing adds another signature row to the signed note. It does not replace the first signature.

The original signed note is not edited directly when you co-sign. If the note needs a correction after signing, use an amendment instead. See Correcting a signed note.

What can't I do here?

Co-signing signed notes has these limits:

  • You cannot co-sign a draft note. The note must already be signed.
  • You cannot use Add my signature after you have already signed that note.
  • You cannot edit the original signed note as part of co-signing.
  • Share with patient is available only for signed notes.

Common questions

Why don't I see Add my signature?

Add my signature appears only on a signed note when your signature is not already listed. If the note is still a draft, sign the note first. If your signature already appears in Signed by, there is nothing else to add.

Can a note have more than one signature?

Yes. The Signed by footer shows one row for each signer and a count of signatures.

Can I change the note before I co-sign it?

No. Co-signing adds your signature to the signed note. It does not unlock the original signed content for editing.

How do I correct something in a signed note?

Use an amendment. Amendments keep the original signed note in place and record the correction separately. See Correcting a signed note.

Can I share a co-signed note with the patient?

Yes, if the note is signed and not already shared. Use Share with patient from the signed note controls or footer. After sharing, the control changes to Stop sharing with patient.
